The Dermagraft Reimbursement Hotline is committed to ensuring that physicians and facility providers receive support in coverage, coding, and reimbursement for Dermagraft.

The Reimbursement Hotline helps answer the following questions:

  • Coverage - Does the patient's insurer cover Dermagraft?
  • Coding - What codes are required for processing of the claim?
  • Reimbursement - What does the insurer pay for Dermagraft and the related codes? Is there a co-payment or deductible?

Additionally, the Hotline provides access to support materials regarding claims processing, including the latest policy information for the third-party payer.

Finally, the Hotline can help research claims that are denied or partially paid and the steps necessary to appeal them.

Coverage

  • 100% of Medicare lives are currently covered for Dermagraft treatment
  • Over 1000 private insurers also cover Dermagraft treatment
  • Up to 8 weekly applications of Dermagraft are covered by Medicare and most private insurers (per FDA labeling)

Dermagraft Coding—MEDICARE

DERMAGRAFT APPLICATION CODES

Physician

There are two new CPT codes effective January 1, 2012 for the physician application of Dermagraft:

  • 15275: Application of skin substitute graft to face, scalp, eyelids, mouth, neck, ears, orbits, genitalia, hands, feet, and/or multiple digits, total wound surface area up to 100 sq cm; first 25 sq cm or less wound surface area
  • 15276: each additional 25 sq cm wound surface area, or part thereof (List separately in addition to code for primary procedure)

NOTE:

  • The CPT code selection is based on the size of the wound, in 25 sq cm increments.
  • Physicians and facilities bill these codes in ALL settings—hospital outpatient department, physician office, and ambulatory surgery center (ASC).
  • There is no global period attached to the CPT codes for the application of Dermagraft.

Facilities

There are two new CPT codes effective January 1, 2012 for the facility application of Dermagraft:

  • 15275: Application of skin substitute graft to face, scalp, eyelids, mouth, neck, ears, orbits, genitalia, hands, feet, and/or multiple digits, total wound surface area up to 100 sq cm; first 25 sq cm or less wound surface area
  • 15276: each additional 25 sq cm wound surface area, or part thereof (List separately in addition to code for primary procedure)

DERMAGRAFT PRODUCT CODE

There are no changes to the product code (Q code series):

  • Q4106: Skin substitute, Dermagraft, per sq cm. [Suggest billing 37.5 or 38 units, rounded]

Private Payers and Medicaid

Some private payers and State Medicaid plans may be slow to adopt the new codes for the application of Dermagraft.
